«Out of the Woods» nació de una carrera contra el reloj. Jack Antonoff, productor y coautor del tema, montó el instrumental con un sintetizador Yamaha DX7 y un Minimoog Voyager para el estribillo, y se lo envió a Taylor Swift mientras ella iba en avión. A los treinta minutos tenía de vuelta una nota de voz con la letra. Fue la primera vez que Swift escribió la letra de una canción sobre una base ya existente, y el resultado fue el tema que ella misma señaló como el que mejor representaba 1989.

El puente cuenta un accidente real: un choque en moto de nieve durante un viaje de esquí que sufrieron Swift y una ex pareja suya, y que acabó con veinte puntos de sutura en una habitación de hospital. Swift había convencido a la prensa sensacionalista de que no lo publicara.

El vídeo lo dirigió Joseph Kahn y se rodó en Nueva Zelanda. Una tormenta severa obligó a retrasar una semana el trabajo, y grupos conservacionistas de la zona acusaron al equipo de incumplir su permiso y poner en peligro a un ave autóctona poco común, algo que el equipo negó. Swift, que aparece huyendo de un bosque mágico, no quiso doble y rodó ella misma las escenas de barro y de nieve; Kahn dijo que «sufrió por el arte». El estreno fue en el especial de Nochevieja de Dick Clark.

El cantante de rock Ryan Adams grabó una versión con aire de country folk para su disco en el que versionó 1989 canción por canción. Y en 2023, tras el conflicto por la propiedad de su catálogo, Swift la volvió a grabar como «Out of the Woods (Taylor's Version)» para 1989 (Taylor's Version); un fragmento de esa nueva versión sonó en el tráiler de la película Migration.
